The following items are some of the paperwork that needs to be completed and returned to the center and updated as needed.

  • Child Care Enrollment – State licensing

  • Heath History and Emergency Care Plan

  • Child Health Report – Child Care Centers

  • Day Care Immunization Record or an electronic record of your child's immunizations

  • Intake for Child Under 2 Years – Child Care Centers (if applicable)

  • Authorization to Administer Medication

  • Alternate Arrival / Release Agreement – Child Care Centers (if applicable)
